End-of-Year Forms

Traditional End-of-Year forms are no longer required and have been replaced with the opportunity for members to share their accomplishments from the year by completing the My 4-H Story document or online form.  Those who have completed the year and submit their My 4-H Story document will receive a completion medal for the number of years they have completed. First year members will receive a first-year pin, second year members will receive a second-year pin, and so on. The Franklin County 4-H Rules that you signed when you enrolled specify the minimum requirements a member must meet in order to receive a completion medal. Among the requirements is the completion of at least one project.  Though the completion of My 4-H Story is optional, it is required to receive your member medal and pin.  The paperwork is due to your club leader by September 16.  Those who complete My 4-H Story by the deadline will be entered into a drawing for some 4-H swag!  4-H members may also have their story featured in the 4-H Newsletter and on the Franklin County 4-H Facebook page.
